This is a really broad scenario, much like the 'Who Cares' or the 'Global Warming' that doesn't change what the disease actually is but changes how the game acts based around your disease, so let me begin...

Scenario Name: Silent World

Description: The world has become separated. All humans on Earth are no longer communicating to each other and every country is now sustaining a much more economic and controlling government. The government, due to canceling all shipment of news and communication the other neighboring countries, has significantly increased their budget, allowing them to spend more money on the main necessities instead of telling the world what is going on. Will a silenced world make your disease much more effective, or will you be eradicated trying to escape it?

Base Elements of Scenario:
- No News articles appear, including those of random news, disease announcements, natural disaster notifications, and research development

- Reasearch speed is increased significantly due to the increased governmental budget

- Planes and ships leave from countries much slower due to the only form of travel being for collection of supplies or shipments

- Research can only be funded by countries that have been infected

- So say for example the United States is infected and say Russia also was infected but China was not, only the US and Russia would aid in research efforts since the fact that the disease is not announced to other countries due to there being no news whatsoever

- No global research efforts put into effect

Disease Type for Scenario: Bacteria, Virus, or Fungus

Starting Country: Any selected

May seem complicated, but really it's just whoever is not infected doesn't know you exist...

Any other disease type will cause for increased infectivity and the sort and extra mutation or game types depending on the type of disease. Those are the basic ones that can suit in the most balanced way depending on the players choice of strategy.
I mean, parasite and prion are similar enough to bacteria that they could work. Bio-Weapon also just has the increased lethality thing, so if you can control that you're set. Nano-Virus would probably be the hardest to do but you could just have it so that once a country is infected, they immediately start max research.
