Every time you play with this, people have different phobias.
Therefore, you may have a harder game due to a group of people who have a phobia of bacteria (Bacteriophobia). This could be an Easter egg scenario unlocked when you leave a country to be the last to be fully infected three games in a row. The achievement could be called [Name of country]ophobia.

That's a really interesting idea! That would definitely be an interesting spin on plagues - and introduce a new concept of people being afraid of the diseases you could infect them with.
