If you buy just the full game on its own, you can unlock the majority of the rest of the game by playing and winning. The only expansion that cannot be unlocked is the scenarios.

If you want to buy the Plague Pack, that will automatically get you all 7 standard disease types, both special plagues, any future special plagues, all current scenarios and any future scenarios.
